We believe no one should have to struggle with pain, be it from a surgery, broken bone, or health condition. At Chesapeake Bay Aquatic & Physical Therapy, we’re always welcoming new patients. Our physical therapists are excited to develop a plan specific to your health and wellness goals and get your journey to physical rehabilitation started.

Policies For Physical Therapy Patients

Whether you are a new or returning patient, Chesapeake Bay Aquatic & Physical Therapy has a few policies you should be aware of. These are in place to enable our physical therapists to provide the best experience possible for all our patients.

Cancellation and missed appointments: At all six of our Maryland locations, our physical therapists understand that sometimes life gets in the way of regular physical therapy appointments. If you need to cancel or miss an appointment, be sure to let your clinic know 24 hours in advance.

Billing: We want your physical rehabilitation to go as smooth as possible. You can help us meet that goal by ensuring we always have your most current billing information on file. Let us know promptly of any changes.

Insufficient funds: We charge a $35 fee for any returned checks. This is payable before your next visit. For any returned checks we charge a $35 fee payable before the next visit. Accounts are past due after 30 days, but the staff at Chesapeake Bay Aquatic & Physical Therapy is always happy to work with you if you’re unable to make a full payment. Let us know your financial concerns, and we’ll work out a plan to make sure your treatment can continue

Maryland Physical Therapy Compatible with your Insurance

Our physical therapists want to make sure you have every opportunity to receive the treatment you need for a happy, healthy life. That’s why all six of our Maryland locations accept most insurance plans. Check here to find out if we accept your insurance provider. If your provider isn’t listed among our accepted providers, don’t hesitate to contact us. Physical therapy is for everyone, and we’ll discuss your options with you.

About Your Visit Patient Education New Patient Forms Insurance and Patient Policies

Fill Out New Patient Forms at Home or In Person

Before you start your physical rehabilitation at any of our Maryland clinics, you’ll need to complete a few forms. These include consent to treatment, medical release, financial policy, and acknowledgment of our cancellation and billing policies.

Please arrive fifteen minutes early to your first visit to allow ample time for filling them out. If it’s more convenient for you, you can find copies of our new patient forms here.

Simply choose your Chesapeake Bay Aquatic & Physical Therapy location and bring the completed forms to your first visit. Either way you go about your new patient forms, we’re excited to help you begin your physical therapy regimen.
